About Us:

Ngati Ruanui Tahua Limited is a w’ānau ora service provider in South Taranaki, offering a kaupapa maori holistic approach to all people in our community. Our operations encompass kohanga reo, early childcare education, w’ānau ora and social services along with our clinical practices.

We are currently seeking an experienced Kai’apai to join our Kahu Taurima (first 2000 days) team, reporting to the Kahu Taurima Lead Coordinator. This role will be based at our Healthcare premise on Hunter Street in Hawera. 

The ideal applicant for this role will have experience working in the W’ānau Ora and/or Social Work fields as they will work with w’ānau to identify their health and wellbeing goals and aspirations, improving access to pathways of care and ensuring wrap-around support for wāhine hapū, māmā, pepē and tamariki. 

Key responsibilities of this role include:

  • Engaging w’ānau in the W’ānau Hāpai comprehensive pathway to support them in setting goals and aspirations which relate to their overall health and wellbeing.

  • Ensuring documentation and data is accurately recorded and up to date at all times e.g. case notes, assessments, care plans, risk assessments, role specific outcomes and contract deliverables.  

  • Supporting and empowering w’ānau into self-advocacy and self-management.

  • Informing and providing support to Māori communities, Iwi and w’ānau.

  • Maintain key partnerships to generate stronger links to foster a sense of collaboration with care providers to enhance multi-disciplinary service delivery. 

To be successful in this role you will:

  • Have W’ānau Ora and/or Social Work experience/competencies. 

  • Preferably have completed two or more years’ experience working with w’ānau. 

  • Have a foundational base of cultural competency including having established community networks and an awareness of w’ānau, hapu and Iwi development. 

  • Operate within a team-oriented environment.

  • Be computer literate and have proven experience using clinical patient management systems.

  • Hold a full and current driver licence.

  • Possess exceptional written and verbal communication skills.
