About Us:

Ngati Ruanui Tahua Limited is a w’ānau ora service provider in South Taranaki, offering a kaupapa maori holistic approach to all people in our community. Our operations encompass kohanga reo, early childcare education, w’ānau ora and social services along with our clinical practices.

We are currently seeking an experienced Kaiawhina to join our Comprehensive Primary Care Team (CPCT), reporting to the Practice Manager. This role will be based at our Healthcare premise on Hunter Street in Hawera, however the incumbent will be required to work for multiple GP clinics in the rohe of South Taranaki. 

This role is focused on the pursuit of Māori and Pacific health gain.  The role involves providing support with the navigation of relevant hauora services, such as the social determinates of health, and beyond health, including education, housing, justice, cultural connection, spiritual and environment. 

The ideal applicant for this role will have established strong relationships across the health and well-being sector. They will have experience working in collaboration with hospital, specialist, primary, community, iwi, hapu, and non-government services to ensure Māori and Pacific people and their w’anau maintain health and wellness (hauora) within the community.

Key responsibilities of this role include:

  • Identifying people and w’anau at greatest risk of complex health issues, assessing and/or reviewing health and wellbeing needs.

  • Providing information and advocacy for people and w’anau by supporting the implementation of care plans.

  • Supporting engagement in activities to promote health and well-being such as screening and immunisation.

  • Working within and for multiple GP clinics in the rohe of South Taranaki.

To be successful in this role you will:

  • Preferably have achieved, or be working towards, a relevant NZQA qualification such as a NZ Certificate in Health & Wellbeing Level 3, or NZ Apprenticeship in Health & Wellbeing (Social and Community) Level 4.

  • Have primary or community healthcare experience.

  • Establish and maintain positive working relationships with people at all levels within related industry, community interest groups, GP Practice team, wider community, secondary care providers, external agencies, and NGOs.

  • Understand the principles of Te Tiriti o Waitangi and be respectful of individual and w’ānau’s personal beliefs, values, and goals.

  • Strong links and experience within Māori communities.

  • Be computer literate and have proven experience using clinical patient management systems.

  • Possess exceptional written and verbal communication skills.
