Ngāti Ruanui Tahua Limited is a w’ānau ora service provider in South Taranaki, offering a Kaupapa Māori holistic approach to all people in our community. Our operations encompass kohanga reo, early childcare education, w’ānau ora and social services along with our clinical practices.

We are currently seeking an experienced Care Coordination Facilitator to join our Comprehensive Primary Community Care Team (CPCT), reporting to the Practice Manager. This role will be based at our Healthcare premise on Hunter Street in Hawera, however the incumbent will be required to work with multiple GP clinics in the rohe of South Taranaki. 

This role is focused on the pursuit of Māori and Pacific health gain.  The role involves facilitating high quality coordination of care as part of the CPCT, for people and whānau with the greatest complexity of care needs which may require their care to be frequently transferred between providers and across services, including hospitalisations.

The ideal applicant for this role will have established strong relationships across the health and well-being sector. They will have experience working in collaboration with hospital, specialist, primary, community, iwi, hapu, and non-government services to ensure Māori and Pacific people and their whānau maintain health and wellness (hauora) within the community.

Key responsibilities of this role include:

  • Identify people and whānau with complex health issues and inequitable health outcomes who will benefit from the coordination of care. 

  • Facilitate completion of a care plan capturing actions required by individuals and whānau, CPCT and other agencies to address identified needs.

  • Assess, plan and coordinate health and wellbeing needs, including the ability to carry out a Needs Assessment.

  • Develop and maintain relationships with key primary care, community, and Māori and Pacific providers. 

To be successful in this role you will:

  • Hold a relevant health professional qualification such as a Registered Nurse or Registered Social Worker, with current annual practicing certification.

  • Have primary or community healthcare experience.

  • Be passionate about working alongside whānau to achieve positive health outcomes.

  • Possess excellent critical appraisal skills and be able to identify the best evidence-informed solutions to clinical and practice questions and issues. 

  • Understand the principles of Te Tiriti o Waitangi and be respectful of individual and whānau personal beliefs, values, and goals.

  • Have strong links and experience within Māori communities.

  • Demonstrate a high level of personal integrity and professionalism, with exceptional written and verbal communication skills.
